Q. Can I make premium payments online?

Woodmen does not offer online premium payments at this time. However, you can have your payment automatically withdrawn from your bank account. Many Woodmen members utilize this method of payment. It eliminates late payments, is very convenient and you may be eligible to receive a premium discount for using this method of payment. To change your payment method, you must fill out a bank authorization form. You can receive a form by contacting your Woodmen Representative or the Woodmen Customer Service Center at 1-800-225-3108. You can also e-mail us with your name, address, certificate number(s) and request.You can also have the pdf form e-mailed to you. You will need Adobe Reader to view the document.

Q. How can I change my address?

The owner of the certificate can contact the Woodmen Customer Service Center at 800-225-3108. You can also visit our Online Service Center and fill out our Change of Address Form, or e-mail us with your name, certificate number(s) and updated address.

Q. How can I change my name on my certificate?

The owner of the certificate needs to mail a signed and dated written statement to: Woodmen of the World/Omaha Woodmen Life Insurance Society Customer Correspondence 1700 Farnam Street Omaha, NE 68102

Please include your name and certificate number on your request. Or, you may contact the Customer Service Center at 800-225-3108 to request a name change form to be mailed to you which you will then need to complete and return.

Q. I no longer smoke or have never smoked. How can I get this changed on my certificate?

The owner of the certificate can contact his or her Woodmen Representative or the Woodmen Customer Service Center at 800-225-3108. You can also e-mail us. Be sure to provide your name, address, phone number and your request to change to a non-tobacco user. Woodmen will then mail you the form and instructions that you will need to complete and return.

Q. Does Woodmen offer a College Savings Plan?

Yes, College Savings Plans are available through our subsidiary, Woodmen Financial Services. Your Woodmen Registered Representative can provide information on the various plans available. Please provide your name, address, phone number, best time to call you and request information on College Savings Plans.

Q. How do I file a death claim?

You can contact your Woodmen Representative, the Woodmen of the World Customer Service Center at 800-225-3108 or e-mail us. We will mail you the forms and instructions. You must fill out the form completely and return it to us.

Q. What are your hours of operation?

Woodmen's Customer Service Center can be reached at 800-225-3108 and is open from 7:00 a.m. to 6:00 p.m. Monday through Thursday, and 7:00 a.m. to 5.00 p.m. Friday, Central Time. Our Customer Service specialists are available to service your call in English, Spanish or Korean.

Q. How can I get a copy of my certificate annual report?

The owner of the certificate can contact the Woodmen Customer Service Center at 800-225-3108 or e-mail us. Be sure to include your name, address, phone number and your request to receive a duplicate annual report.

Q. When do certificate annual reports for traditional and Universal life insurance, and annuities get mailed?

Traditional and Universal Life certificate statements are mailed each year by the 15th day of the anniversary month. Annual Reports are mailed each January for all annuity certificates. Mid-year reports are mailed in July for the following types of Annuity certificates with values of $5,000 or more: Flexible Premium Deferred Annuity; Single Premium Deferred Annuity; Non-Life Single Premium Immediate Annuity. These include Basic Annuities, IRAs, ROTHs, SIMPLEs, SEPs and TSAs.

Q. How can I get Woodmen's Discount Prescription Drug Card?

The owner of the certificate should call 877-223-4244 or e-mail us. Be sure to include your name, address, phone number and certificate number, if possible.

Q. How can I get information on a deceased relative for genealogy purposes?

Due to federal and state privacy laws, Woodmen will not disclose the personal information of deceased individuals unless it is in relation to a claim or a potential claim. You may be able to find information about a deceased relative from the Family Search Centers, including the Family History Library in Salt Lake City, Utah. Their telephone number is 1-800-406-1830.

Q. How can I order a duplicate tax form?

The owner of the certificate can contact Woodmen's Customer Service Center at 800-225-3108 or e-mail us. Be sure to provide your name, address and phone number, and request a duplicate tax form.

Q. When will I receive my tax information?

The 1099 forms are mailed each year no later than January 31. Tax information is also available by calling our Customer Service Center at 1-800-225-3108 or e-mailing us.
